Guidelines for Grant ApplicationsAny contributions of funds or materials approved by CDS are limited to bona fide charitable 501(c) (3) organizations, educational groups and organizations dedicated to the art and science of dentistry through either professional or public initiatives primarily in the Chicago area and state of Illinois. Grants to charities are limited to a maximum of $10,000. Grant Application Form RestrictionsThe Chicago Dental Society does not fund the following:
Applicants should be aware that the CDS should not be considered a continuing source of annual funding. Responsibility of Funding RecipientThe recipient of a grant must use funds awarded for the specific purpose for which they were intended. Any funds not used must be returned. Within one year, CDS requires a written evaluation and a detailed accounting report for the grant awarded and how the grant funds are utilized. The evaluation of your project will influence the likelihood of future grants to your organization. A site visit may also be conducted. We realize that there are many fine organizations that provide support for these programs. Application ProceduresThe Annual Grants Application must be completed and the request submitted in writing on the organization's letterhead. Fax or e-mail requests will not be accepted. DeadlinesThe deadlines to submit typed requests and application forms are April 1 and November 1. Completed grant applications will be forwarded to the Grants and Donations Committee for review. After reviewing an application, the Committee may decline or recommend for approval by the Board of Directors. Final grant decisions will be announced approximately eight weeks after each deadline.
